This drawing is a reference to the Fleetway Sonic Comics, aka "Sonic the Comic."
In this universe, Dr. Robotnik once stole Sonic's powers. Consequently, the doctor took on the physical features of the hedgehog. His clothes and mustache turned blue, quills grew out his head and back, and his feet turned red.
In order to reobtain Sonic's powers, Robotnik attempted to recreate the accident that turned Sonic blue and increased his speed. However, his plans were foiled by Amy Rose.
This wouldn't be the only time, Eggman has obtained Sonic's powers. Look up "Sonic Cookie Eggman" from the "Sonic Boom" cartoon.
In this drawing, Dr. Eggman has obtained the powers of the westernization of Sonic the Hedgehog. His top has turned blue, quills have grown out of his back, Sonic's sneakers have appeared on his feet, and a blue mohawk has grown out of his head.
