Here I have my chainaxe head.  I've built up the head using PVA foam.   I then placed the skull over the engine mount, next to it is the battery pack mounting.

I'm going to angle the edge of the foam near the edge.  

Should I make it symmetrical and add a skull to the other side too?  or add some other decoration

40K style usually uses decorative elements to hide working bits that need covering. A little asymmetry give that rough-edged look that says "instrument of death". Purity seals, medallions or painted scenes could fill in the blank on the other side; depending on your skill or whoever you can talk into helping you out.

purity seals wouldn't fit since I'm going for a more Orky look to go with my cyber claws.  

Originally I was going to have it look like a spike was driven through the blade pinning two skulls to it.  but since the motor fits inside the skull no need for that.   I'm going to have some cabling running out of the skull down to the trigger as well so the skull will be made to look like part of the engine (maybe orange led's set in the sockets too.)  But I don't want the other side bare.   build it up and I'll come up with something to decorate it.  I have some more bones

For Orkyness you need something mechanical like a gear box (a shifter lever?) or maybe a tribal glyph. I don't know if it can be done, but how about an exhaust pipe running from the skull to the other side? Feed it into a tube with different sized holes in a line, then a smaller output. Tell them you're with the band and it's a tuned exhaust.

I have been thinking of an exhaust system  

Piping running at slight angle from a box bolted to the other side would fit.  I had wanted to set up a soun system for runnint the motor I even bought some sound boards... but I can't figure out how to actually use them
