If you read it's article with it's provided one image, you can see them dodge the question of gender by always referring to it as Mal0 instead of any kind of pronoun. The image suggests that its so far out of the realm of what you'd consider human that gender is not even considered cause you are literally looking at something akin to a vendigo. A skull with eyes in it.
The log with one of the so called victims of it (Capcone) has it referenced as "he" but also does not seem to be doing so due to any specific reasons. Its just a terror inducing monster.
IF you look past all of that however, its easy to interpret its nature as the quintessential dandere/shundere gf trope. Shy, silent, but always around, caring, trying to comfort but having zero idea how to do so, which in a nutshell seems to be the intended purpose of Mal0 to begin with.
