Frisk, Asriel, Chara, and Monster Kid are doing little potty dances because they have full bladders and desperately need to go to the toilet. Unfortunately, Toriel's currently in there doing cleaning, so they're unable to use it.
They can't hold it in though and they're currently wetting their pants. As they all lose control, Toriel peers out from the bathroom door to see what the commotion is and kindly let's them know that she's cleaning the bathroom.
There's going to be some more significant cleaning after this is done too it would seem.
