Portal heads or "spaghetti men" as some units nickname them, are actually small holes in reality through which the tendrils of a minor Outsider are poking out. What appears to be an anthromorphic figure is actually the hands or tendrils of something much larger, a fact betrayed by their normally awkward, gravity-defying movements, which betray that they do not actually obey rules of physics such a gravity.

Portal Heads as intelligent foes capable of planning, tactics, coordination, and other advanced combat techniques and as such, will often be found forming small combat groups or will serve under the employ of a major Outsider, under a wide variety of roles. They are most often found under the employ of the Once And Future King but there have been confirmed sightings of them within the ranks of Ellipticon, Nox'hhrvl, and Vertigo. Some of them have also been known to serve especially daring (and foolish) hellish dimensions and some higher-level maloevent divine entities.

Portal heads do not tend to carry weapons on their person but being the fingers of beings that can sometimes be as large as a gas giant-class planet, the level of force that they can bring to bear when they physically strike something can be truly tremendous. As small tendrils pretending to be the shape of a humanoid, its owner can also deform them, extend them, and generally freely change their shape to match the situation.

Most portal heads are also known to be low-level reality warpers, often in the form of being able to unleash what appears to be screaming ghosts at their foes as projectiles. Those projectiles are nowhere near as destructive as their physical attacks and despite their accuracy (they are homing), struggle with ignoring dense materials or dealing damage to inorganic matter (such as armor or mechanical beings).

This does not mean portal heads cannot be dangerous at range ; many portal heads are known to make a shape with their "hand" that mimic a gun and then to fire arcs of lightning, often at intensities strong enough to melt steel and this, at ranges and with accuracy that is often beyond visual range for most organic species. Portal heads fed targeting data by other void predators have been known to manage surface-to-orbit shots with those lightning blasts, making those particular portal heads a priority target during orbital bombardment.

Portal heads may appear fragile and may be scared off via inflicting pain on them through physical damage but this is unreliable. Remember that those are nothing but a very small part of a large being that may not necessarily feel or receive much damage from having such a small part of itself destroyed. Thus, the most effective method of dispatching those foes is to target the vortex from which the main frame is sticking out ; if the vortex can be forcefully slammed shut, the portal head will instantly disappear.
