OK here's what I know about Natani: His physical body perished in some sort of accident or battle, and then his consciousness was transferred into his sister's body. It's a male consciousness inside of a female body, which is why he tries to hide his breasts with that magical chest wrap of his. BUT Natani is still biologically female, which makes it even hotter for me because of the awkwardness involved. It's a dude trying to hide the fact that he has tits and a puss, and that is amazing and I will die on that hill.

Actually, Natani was born a girl. Her soul was severely damaged by an attack of black magic. As a last resort to save her life, her brother procured a, shall we say, "experimental" treatment that bonded their souls together to fill the "gaps" in hers. (This also resulted in their telepathic link.) One of the parts that was damaged was her gender, so it was filled by her brother's gender.
An additional factor seems to be that she sees females as weak, partially because of wolf keidran culture, and partially because, when they lost their parents at a young age and were struggling to survive, her brother wanted her to be tougher than the little girl that she was.
More recently, she fell into a coma when her brother was magically cut off from her, but she managed to wake up (while still cut off from him) by at least partially re-accepting the feminine part of her mind that had apparently been around all along, and shedding some insecurities in the process.

It seems to me that Natani has been gradually remembering and embracing the femininity that she had before it was damaged by a variety of mental trauma (see my reply above). She told Adelaide that it was about not being seen as weak, but Adelaide pointed out that female and strength aren't mutually exclusive. She's increasingly opening up to a relationship with Keith. She managed to wake up while magically cut off from her brother by partially re-accepting her suppressed feminine self, shedding quite a bit of insecurity and decreasing her dependence on her brother.
I'm not saying you're wrong, but I am saying that if it's possible for each of us to interpret it so differently, then it still seems to be pretty ambiguous.

Given the context of the conversation (particularly p. 931) and the rest of the comic plus the way in which Natani seems to understand the words "masculine" and "feminine", I always interpreted that part to mean that Young Natani was just naturally a tomboy, particularly since she "didn't mind being a girl". Young Natani never specifically said she preferred being seen as a guy, just that she liked it (i.e. maybe just that she got to be less of a girly girl).
Additionally, Young Natani has a female body in the mindscape, which would seem to mean she sees/saw herself as a girl, but I suppose that might just be due to how Current Natani sees her.
If the comic were to end right now, I'd be inclined to agree with you, but as it is, Natani's storyline doesn't seem to be resolved yet and she seems to be gradually learning that she can be a girl and still be a warrior.
I concede that the comic gives mixed messages, but that's why I think it's ambiguous. If that's not Tom's intention, then I think he needs to be a bit more explicit. I don't think I've ever even seen him use a pronoun for Natani in any of the Patreon sketch captions.
