Four-point restraints and a [breathable] ball gag. Then you tease your captive for minutes (not hours; they're usually too little) by getting really close to sensitive areas just to maximize attack-anxiety. Then, there's the few times in a torture session that you reward the captive sprinkling bits of pixy-sticks into the holes of that perforated gag.
