wooh! As for the "how often do you open" question, I unfortunately don't have a good answer to that xD; I take in a lot (I don't make anyone pay before I'm ready to work, so it's always possible to get off the list) and then re-open when I'm done with the current list.

It has taken me MANY years to get where I am now. I've been doing commissions full-time for over 10 years. What I think is important is to have a page with prices and examples. Having a clear list of what you are ok with/not ok with drawing is also very useful, so people won't have to ask questions. Starting small is also useful. Things like raffles/requests will also help with practicing drawing other people's characters, as well as give you samples for people to look at. Another advice would be to not be afraid to promote your commissions (i.e: posting a "commissions open" illustration) I think patience is the most important, though. And not giving up even when there aren't many requests coming in.
