Punk Heart Bear is a purple care bear with white and indigo accents. They are wearing a bad ass leather punk jacket with shoulder spikes and studs on the open jacket flaps. They have two earrings in their right ear, one yellow and blue. There are numerous belts and zippers on it. PHB is wearing a disposable diaper with a red landing zone with musical notes on it. The diaper has black sides.

They have a determined expression on their face with a black paci in their muzzle and a star hovering above them, indicating that they are a star child.
