Either 682 realized what her powers were and managed to evolve a defense to keep from going berserk at her (but can't bypass the heart attack from attacking her and chooses not to attack), or he genuinely cares about her. I could swear I recall that 682 asked about her well being on at least one occasion.

As for the rest, I'm nowhere near far enough down the SCP rabbit hole to start theorizing about lore. That said, could be that 053 simply lacks fine control of her power due to her youth. As she gets older, maybe she'll shed the rage aspect and just be able to induce heart attacks at will.

SCPs have a tendency to treat the laws of time and space less as rules and more as things that happen to exist that they interact with. 053 may be the firstborn, but the trick is, who can say when that was, if 682 is a sibling, or if such things even exist among SCPs as we, ordinary humans, understand such a concept?
As an aside, that question is why I try not to delve too far into stuff like this. If I let my mind wander down a road like that, I may lose months of my life on chatboards, and I badly need to spend my time more wisely as is.
