Hmm, the idea of a story springs to mind. Leo gets injured badly, and Donnie had just finished a project known as 'The Recovery Pod'. It has the patient suspended in some kind of fluid (could be water, or a healing mutagen solution, whatever), and it heals them while they sleep. In the picture, Raph is waiting while holding Leo's mask, which says he's either really worried, or he somehow blames himself (maybe Leo took a hit for him or something)
