So I gave him a bad trip, it's creepy, and it pays homage to Saw 2's needle pit. I actually drew up a large syringe all detailed and everything, copied the lines a few times and colored them with different liquids, then shrunk it down to size and mass multiplied the layers to get the huge pile on the pit. The witnesses in my stream for this image can attest!
