As for the spigot in her eye: it's based off the old Valve logo and splash screen that popped up after launching many of their earlier games. They don't use it anymore, and have instead replaced all of the logos with a bald guy with a spigot in the back of his head (although there's a creepier version of that in later games...)
The background is supposed to resemble the old HL1 box art.
